A systematic review of mealtime recommendation interventions provided to carers of school-aged children who have neurodisability and oropharyngeal dysphagia

Abstract

Introduction
Children with neurodisability and oropharyngeal dysphagia have an increased risk of premature death1,2. Speech & Language Therapists provide mealtime recommendations to optimise safe nutrition and hydration, which carers may find difficult to implement. There is limited guidance for clinicians regarding which mealtime recommendations to select and how to support implementation by carers. This systematic review will identify and evaluate current evidence on mealtime recommendations and implementation strategies.

Methods
We searched 11 databases alongside manual searches in August 2021. Two reviewers independently screened records against inclusion/exclusion criteria using Covidence software. Eligible studies reported on school-aged children with neurodisability and oropharyngeal dysphagia carer mealtime recommendation interventions against any comparator. They will be quality assessed using the NIH quality assessment tool, PEDro, ROBiN-T or CASP critical appraisal tools. A meta-analysis will be performed if data permit, however we anticipate a narrative synthesis due to study heterogeneity.

Results
Database searches found 7682 records. After de-duplication 2603 records were screened with 108 progressing to full text review, plus an additional 22 via citation checking. 35 records were included in the review. Descriptive statistics will be used to describe child, carer and professional study participants and other key aspects. The synthesis will report on details of the interventions including behaviour change techniques used, intervention effectiveness, outcomes and tools used for measurement.

Conclusions
This work will provide valuable clinical information on content and effectiveness of the under-researched area of school-aged dysphagia mealtime interventions. Findings are part of a wider project aiming to create a mealtime recommendation toolkit for families of children who need assistance with eating and drinking.
